On the heels of the success of our email/web forms, I decided to build on the concept by designing on form that would write the data to an SQL database. While the concept was mine, much of the programming has been accomplished by another ITSS staff member and a student employee: Dan Burrows and Jenna Pederson. My goal was to improve the form in the following ways:

  • Simpler format with problems consolidated into one pulldown list
  • Still "publicly" available (UMD domain)
  • Still generates email to appropriate players
  • Also creates SQL database entry - which is searchable and allows better tracking of data in the short term and over time.
