This step is also easy to skip. After the "excitement" of completing a health project, it is tempting to not follow through with the evaluation plan. Maybe you need to plan a "wrap-up" meeting/party to gather all the partners together. This may be a good time to consider what recommendations the group has to improve the outcome of the project.

  1. Were the goals of the project met?
  2. Can a change in health status be measured immediately, in 1 year, in 5 years?
  3. Were there adequate resources available to conduct the project?
  4. What would you do differently next time?
  5. Did anything unexpected (surprising) occur as a result of the project?
