Health & Social Services Wheel

Definition:

This category tends to contain the largest amount of data that health profession teams desire. You may prefer to organize data into subcategories, such as:

Health Services: Social Services:
Physical
Spiritual
Mental
Economic
Housing
Support groups

Essential Data:

Consider the structure (what types of services), and the process (how accessible).

Examples of Questions:
  1. Where do members of the community obtain their prescriptions?
  2. Who in the community are the informal health care "consultants", offering advice, education, and opinions?
  3. How many families are enrolled in the WIC program and where do they pick up their vouchers?
  4. How far do people have to travel for health care, to fill a prescription, dental care, eye care, etc?

Did You Know?

Some rural (frontier) regions do not have phone accessibility. Some counties don't even have dental services.

Have you ever wondered what it is like for a community to have one ambulance? What happens when they are on a transport?
