Reporting of Incidents of Bias, Discrimination, Harassment, and Law/Policy Violations
There are two versions of the Bias, Discrimination, Harassment Reporting Form (Complete only one version).
The University of Minnesota is committed to providing a non-discriminatory work and educational environment. Board of Regents' policies prohibit discrimination on the basis of race, color, creed, religion, national origin, gender (including gender identity/expression), age, marital status, disability, public assistance status, veteran status, or sexual orientation. The UMD Department of Human Resources and Equal Opportunity (HR & EO) implements the policies by working to create an inclusive environment by helping to educate our community and resolve conflict.
Reporting incidents is important, so that you or someone you know can receive support, we can work to prevent further acts of bias, discrimination and harassment, and we can improve campus climate.
Reporting of incidents has been re-emphasized and expanded to include incidents such as offensive graffiti, anonymous phone calls and emails. These contribute to a hostile campus climate and need to be reported.
The UMD Department of Human Resources and Equal Opportunity assists students and employees with concerns about possible discrimination. If you have experienced or observed potential discrimination, an Title IX Investigator can discuss the issues, help resolve the problems, investigate, or provide educational programming. We can provide the best assistance by meeting in person, or at least discussing the situation in a phone call. To make an appointment, call 218-726-6827.
