If you are in a community-based learning course at UMD, please use any of the forms below. Your instructor may require you to use some forms, while others might be optional.
Online registration, Parts 1 and 2 - This is where students will complete two parts of an online registration. We collect basic contact information, course information, and the community site(s) at which students are serving for their course requirement.
Dress Code Agreement - As a reminder, this is the agreement you sign off on upon completing our online registration. Please review it as needed.
Student Contract - As a reminder, this is the contract you sign off on upon completing our online registration. Please review it as needed.
Volunteer Hours Log - You may use this to track your service hours at your site. If you are serving at more than one location, we suggest using one sheet per service site. Our office does not track or collect these sheets - they are for your own use.
Student Evaluation - Your site supervisor (the person supervising you at your service site) will use this form to evaluate your performance. Your instructor (UMD faculty teaching your course) may ask that you scan a completed form and upload it to a course site, or they might request that it be delivered to them in a sealed and signed envelope. It is up to the instructor how they would like to receive a completed evaluation form.
